Normalize session watcher Event v2 turns and questions

Purpose

Normalize session watcher events for the modular orchestration follower (O1).

Changes

• Preserve Codex response_item phases and emit one turn_complete result with turn ID, outcome, exact reply, and error details.
• Extract question events from Codex and Claude question tools.
• Add transcript creation, origin, and bridge-parent facts to discovery.
• Keep the existing event fields and text rendering, adding Event v2 fields.

Validation

• npm run verify passed.
• python3 packages/tmux-bridge/scripts/event-v2-test.py passed.
• Replay of 27 local rollout transcripts: 157 completed turns, 154 exact replies, 3 errors, 0 mismatches.
• git diff --check and public/private text scan passed.

Rollout

Deploy the merged agent-mesh release used by the bridge, then verify installed watcher output on live transcripts. O2 follower will consume this parser through events_for.
