#1920 r14: malformed-result refusals name the wrong fault

Codex round 14 on PR #1920 (head 86caf26), should-fix. The refusal text attributes every malformed count to an assistant/user event whose message is not a mapping; a result whose usage is not complete accounting is another cause. Fix the text and CLAUDE.md. Fix in #1920.

Fixed in acc3ead on PR #1920: the malformed refusal and CLAUDE.md name a result whose usage is not complete accounting as a cause. Closes when #1920 merges.
