feat: 0.18.0 planning lifecycle release candidate

Summary

• Add opt-in lifecycle history, guarded writes/recovery, retired-ID protection, and preserved review debt.
• Add purpose-aware planning review and stale KPI evidence indicators without changing SOT 1.0/1.1.
• Preserve historical failures, publish only synthetic audit evidence, and align usage/release documentation.

Verification

• Latest main baseline and integrated 0.18.0 npm run check:all: PASS.
• Logic matrix 92/92, shop-tree holdout 18/18, process resilience 12/12.
• Independent integration review: no Critical or Important issues; minor documentation correction applied.
• Tracked path/privacy and Markdown links: PASS.

Release boundary

Candidate merge only. Do not tag until independent fresh Claude/Codex full-mode generation matches the final runtime digest and local plus GitHub host acceptance gates pass. Journal remains opt-in; automated checks do not confer product approval.
